I understand and agree Halite Solutions Group LLC is not a healthcare professional and nothing in this Toolkit constitutes medical advice. Further, this Toolkit is not a comprehensive, complete, or exhaustive review of my medical information and history, it is merely a brief form to help people with cystic fibrosis to record their medical information and to present that information to healthcare professionals as supplemental information. It is not designed to replace the judgment of a healthcare professional, such as a physician, or to be used to diagnose or to treat a condition of cystic fibrosis. Only a licensed physician can diagnose and treat such a condition.
I, my personal and legal representatives, heirs, successors and next of kin will not make any claim against Halite Solutions Group LLC, or any of its present or former members, managers, employees, agents, attorneys, insurers and representatives and their respective successors, heirs and assigns or any volunteers, for injury, damage, death or any other loss arising from or related to this Toolkit.
